The portal is available to current patients of The West Durham Family Health Team.

Patients will be sent an invitation by email to join the portal. Please ensure that we have your current email address. Each patient must have a unique email address. Please fill out the form below and complete all fields. By providing us with your email address, you agree to the following:
- The emails transmitted will contain personal health information. The office cannot guarantee the security of any email message transmitted outside of its email system.
- The email communication cannot contain any urgent or emergency concerns. Urgent concerns must be called into the office. IF YOU ARE EXPERIENCING AN EMERGENCY, YOU MUST CALL 911 OR GO TO THE NEAREST HOSPITAL.
- It is the patients’ responsibility to monitor email communications and to promptly notify the office if your email address changes.
